Subject of the letter: CONSENT LETTER

Attach:- copy of birth certificate

Consent letter signed by both parents

2 passport photos

Original and copy of payments

PASSPORT RE-ISSUE

LOST PASSPORT

 Attach:- Sworn Affidavit

Police Abstract

NB: CASES OF LOST PASSPORT, CHANGE OF NAME, CHANGE OF DATE OF BIRTH, SHOULD COME TO EMBASSY FOR CLEARANCE BEFORE PAYMENT.

Kindly know that presently from 20 September 2022 till further notice, we can only issue 32 pages passport booklet.
